Although they both mean to have the attention or focus on something interrupted or disturbed, they are very different.
"Defocused" is more common in the UK than in the US. And it is almost always used when referring to optics, lenses, microscopes, or cameras.
"Unfocused" can be used to describe these technical aspects, but it can also be used to describe thoughts or goals.
Ex 1: Even the slightest touch defocused the image seen through the telescope.
Ex 2: She stared at the image so long that her eyes became unfocused.
