tauschen means to swap things
For example: if you want to swap a card with a friend (you want his card and he want your card)
= „Sollen wir Karten tauschen?“
There is a word which means „Austausch“ for example when you ask somebody for his number you swap numbers = „Können wir Nummern austauschen?“
Wechseln means to change something
For example: when you want to change the wheel of your car. You can say „Ich möchte gerne meinen Autoreifen wechseln“ and if you talk to yourself you say „Ich Wechsel meinem Autoreifen“ you can use the word „auswechseln“ as well.
Ändern means to change something up to something else or to create something new. For example: „Ich werde meinen Handycode ändern.“
