Zusammenfassung
Betablocker-, Kalziumkanalantagonisten- und Digitalisglykosid-Intoxikationen sind häufige Vergiftungen. Schon bei einer geringen Überdosierung können lebensgefährliche Komplikationen auftreten.
Das klinische Bild der Betablocker- und Kalziumkanalantagonisten-Intoxikation mündet in einem kardiogenen Schock. Bei einigen Betablockern und zentralen Kalziumkanalantagonisten (Diltiazem, Verapamil) kann es zudem zu höhergradigen AV-Blockierungen und weiteren Herzrhythmusstörungen kommen. Peripher wirksame Kalziumkanalantagonisten (z. B. Amlodipin) führen zu einem primär vasoplegischen Schock. Digitalisglykosid-Intoxikationen zeigen zudem nichtkardiale Symptome, hier sind als Besonderheit Sehstörungen zu nennen. Elektrokardiografisch zeigt sich häufig eine Abflachung oder Inversion der T-Welle und eine muldenförmige ST-Strecken-Senkung. Eine Sinusbradykardie ist das häufigste Symptom, es kann aber zu vielfältigen Tachy- und Bradyarrhythmien kommen.
Alle mittelschweren und schweren Kardiaka-Intoxikationen sollten auf einer Intensivstation versorgt werden. Die Vorhaltung einer Salvage-Behandlung mit extrakorporalen Ersatzverfahren (insbesondere der venoarteriellen ECMO-Behandlung) ist bei schweren Vergiftungen mit therapierefraktärem kardiogenem Schock ratsam.
Eine gastrointestinale Dekontamination mit Aktivkohle sollte durchgeführt werden, wenn eine mittelschwere oder schwere Vergiftung zu erwarten ist – unter Beachtung der Kontraindikationen und der Latenzzeit zwischen Einnahme und Applikation (siehe Kap. „Antidote“). Die initiale Stabilisierung sollte bei Bradykardie mit Atropin erfolgen, Norepinephrin und Epinephrin sind die Katecholamine der Wahl. Eine kardiale Schrittmachertherapie kann bei Patienten mit Bradyarrhythmien notwendig werden.
Bei schweren Betablocker- und Kalziumkanalantagonisten-Intoxikationen ist eine Hochdosis-Insulin-Glukose-Therapie indiziert, bei Kalziumkanalantagonisten-Intoxikationen sollte zusätzlich Kalzium verabreicht werden. Bei peripher wirksamen Kalziumkanalantagonisten kann bei therapierefraktärem Schock Methylenblau eingesetzt werden. Für die Gabe von Glukagon gibt es wenig Evidenz, die Hochdosis-Insulin-Glukose-Therapie ist ihr überlegen. Eine Lipid-Rescue-Therapie kann im therapierefraktären kardiogenen Schock oder im Rahmen einer Reanimation angewandt werden.
Digitalisglykosid-Intoxikationen lassen sich effektiv mit dem Digitalis-Antitoxin mit Anti-Digoxin-spezifischen Fab-Fragmenten behandeln. Auf eine engmaschige Kontrolle der Elektrolyte ist zu achten und Elektrolytstörungen sollten ausgeglichen werden. Magnesium findet als membranstabilisierendes Mittel bei Torsade-de-Pointes-Tachykardien Anwendung.
