Zusammenfassung
Bei einem 7-jährigen Mädchen wurde 3 Monate nach Beginn von Nackenschmerzen und Schmerzen zwischen den Schulterblättern aufgrund progredienter neurologischer Ausfälle eine Magnetresonanztomographie (MRT) der Wirbelsäule durchgeführt. In dieser zeigte sich eine das Myelon hochgradig einengende Tumorformation dorsolateral zwischen dem 4. und dem 6. Brustwirbel. Die histopathologische Aufarbeitung des im Rahmen einer Notoperation entnommenen Gewebes ergab die Diagnose eines Osteoblastoms. Die Patientin erholte sich nach der Operation vollständig, und nach kompletter Tumorresektion ist in den rezenten Nachkontrollen kein Rezidivwachstum zu verzeichnen. Da Rückenschmerzen im eigentlichen Sinn bei Kindern eher selten sind, liegt ihnen häufig ein organisches Korrelat zugrunde. Bei der Abklärung derartiger Beschwerden muss ein breites Spektrum an Differenzialdiagnosen berücksichtigt werden.
Abstract
During a 3-month period of neck pain and pain between the shoulder blades, a 7-year-old girl progressively developed neurological deficits in the lower extremities. Magnetic resonance imaging (MRI) of the spine revealed a high-grade penetrating dorsolateral tumor formation between the fourth and sixth thoracic vertebra which compressed the spinal cord. An emergency operation was performed and histopathological examination of the resected tissue established the diagnosis of an osteoblastoma. The patient recovered completely after the operation and following complete tumor resection a recently carried out follow-up control with MRI did not depict any signs of tumor relapse. As back pain is extremely rare in children it is mostly caused by a relevant pathoanatomical correlate. Therefore, a wide range of differential diagnoses must be kept in mind for the clarification of such complaints.
