Zusammenfassung
Als Zweihöhleneingriff bietet die Ösophaguschirurgie im Vergleich zu anderen viszeralen Operationen die variabelsten Kombinationen aus verschiedenen technischen Möglichkeiten. Offen, minimal-invasiv, roboterassistiert, „Hybridvariationen“, eine detaillierte Beschreibung der technischen Schritte dieser Operationen sowie die Analyse schwieriger intraoperativer Situationen und deren Lösungen werden in diesem Kapitel ausführlich behandelt. Laut den Kapitelautoren sind die Lage des Tumors im oberen Ösophagusdrittel, kleines Ösophaguskaliber, „downstaged“ cT4bNx-Tumoren, abdominelle Voroperationen mit Adhäsionen der gastroepiploischen Arkade bzw. Voroperationen des Magens, die ggf. ein Koloninterponat erforderlich machen, größere intrathorakale Voroperationen bzw. Fibrothorax, obere mediastinale Lymphknotenmetastasen, schwere Kyphoskoliose, ausgedehnte neoadjuvante Chemoradiotherapie bzw. Salvage-Chirurgie nach langem zeitlichem Abstand nach neoadjuvanten Therapie sowie BMI >35, prädisponierenden Faktoren für Entstehung der schwierigen intraoperativen Situationen.
