All medical records are reviewed by the chairman of the Cleveland Clinic Heart and Vascular Institute to determine which National Network hospital and surgeon will provide the best clinical outcome for each patient’s specific needs.

Kohl’s Cardiac Care Program patients work with a dedicated patient care coordinator before they even arrive at a National Network facility. The patient care coordinator is the main contact for patients and their families throughout their experience at the National Network facility and maintains a relationship after they return home.
