Zusammenfassung
Die Rhinopathia gravidarum oder Schwangerschaftsrhinitis ist eine die Schwangerschaft begleitende Behinderung der Nasenluftpassage ohne Hinweis auf eine Infektion, Allergie oder Raumforderung und zeichnet sich durch das rasche Verschwinden der Symptomatik nach der Geburt aus. Etwa ein Viertel aller Schwangeren ist von diesem Krankheitsbild betroffen, welches zu jedem Zeitpunkt der Gravidität auftreten kann. Pathophysiologisch werden v. a. hormonelle Einflüsse verantwortlich gemacht. Raucherinnen sowie Patientinnen mit vorbestehender perennialer Allergie und chronischer Sinusitis besitzen ein erhöhtes Risiko daran zu erkranken. Mögliche Folgeerscheinungen schließen Mundtrockenheit, Schlafstörungen und Kopfschmerzen ein. Weiterhin ist das Risiko, an einer Nasennebenhöhlenentzündung zu erkranken, deutlich erhöht.
Nachfolgend sollen die Symptome, die Pathogenese und mögliche Therapieoptionen der Schwangerschaftsrhinitis in Form eines Stufentherapiekonzepts vorgestellt werden.
Abstract
Pregnancy specific rhinitis, a long lasting nasal congestion without signs of infection, allergy or tumor occurs in about 25% of pregnancies. It can start at any time during gestation and disappears completely within 2 weeks of delivery. Besides smoking and sensitisation to house dust mites, chronic sinusitis is one of its major risk factors. The consequences of pregnancy rhinitis include dryness of the mouth, headaches, sleeping disorders and the induction of sinusitis. Hormonal influences are most likely, however, a definitive pathophysiological concept does not exist. Besides its symptoms and pathogenesis, various treatment options for pregnancy rhinitis are presented and a stepwise pharmacotherapeutic strategy is developed.
