Zusammenfassung
Seit der Entwicklung der modernen Thoraxchirurgie haben sich zahlreiche unterschiedliche operative Zugangswege in der Thoraxchirurgie herausgebildet, die abhängig von der Indikation und Dringlichkeit der Operation komplementär Einsatz finden. Dieser Artikel soll eine Übersicht über aktuelle Zugangswege sowie deren Indikation und Durchführung geben.
Die Thorakotomie ist der Standardzugang für die offene Chirurgie. Trotz der erfolgreichen Etablierung minimalinvasiver und robotischer Techniken am Thorax behält die Thorakotomie ihre unbedingte Berechtigung als wesentlicher Zugangsweg insbesondere in Notfallsituationen, da sie je nach der individuellen Expertise des Operierenden einen sicheren Zugang bietet, der im Falle eines akut notwendigen Umstiegs von minimalinvasiven auf offene Verfahren auch die bestmögliche Blutungskontrolle bei Major-Blutungen gewährleistet. Aufgrund des geringeren Zugangstraumas und der damit verbundenen geringeren peri- und postoperativen Morbidität sowie einer nachweislich rascheren Rekonvaleszenz zählt die videoassistierte Thorakoskopie heute zu dem Standardzugang erster Wahl insbesondere in der onkologischen Thoraxchirurgie. Die robotisch assistierte Thorakoskopie ist insbesondere bei mediastinalen Eingriffen überlegen und gewinnt zunehmend Bedeutung bei thorakalen Operationen. Zur Resektion lokal infiltrierender Tumoren des vorderen Mediastinums oder der oberen Thoraxapertur haben sich teils variantenreiche Zugangswege entwickelt. Aufgrund der unterschiedlichen Vor- und Nachteile der klassischen mediastinalen Zugangswege werden diese heutzutage vielfach kombiniert angewendet und durch den Einsatz von minimalinvasiven Techniken ergänzt.
Abstract
Modern thoracic surgery encompasses a multitude of different and specific surgical approaches. This article aims to give an overview of contemporary surgical techniques for access to the thoracic cavity and mediastinum, their indications and implementation. Thoracotomy is the classical approach in emergency situations and for the resection of large tumors or in cases of pleural adhesions due to previous surgery. Despite the successful implementation of minimally invasive techniques, thoracotomy remains an essential part of thoracic surgery especially as it serves as a safe technique in cases of major bleeding and probably necessary conversion from minimally invasive surgery to open procedures. Nowadays, video-assisted thoracoscopy is the procedure of first choice especially in oncology because of its small access-related trauma and low perioperative and postoperative morbidity and rapid recovery. Robotic-assisted thoracoscopy is the superior procedure, particularly for mediastinal tumors and gains increasingly more importance for thoracic operations as well. Resection of tumors infiltrating the anterior mediastinum or upper thoracic aperture requires sophisticated techniques. Today, these techniques are often combined with minimally invasive procedures.
