Zusammenfassung
Die bedarfsadaptierte Sekretion und Wirkung von Kortisol ist essenziell für die Homöostase. Ein Kortisolmangel bei kritisch kranken Patienten („critical illness related corticosteroid insufficiency“, CIRCI) und der Nutzen einer Therapie mit niedrig dosierten Glukokortikoiden wird seit Jahren kontrovers diskutiert, insbesondere bei Patienten mit septischem Schock. Die Revision des Schocks durch Hydrokortison wurde in mehreren Studien nachgewiesen, jedoch erscheint ein Überlebensvorteil im Gesamtkollektiv zweifelhaft. Aktuelle Leitlinien sehen in der adjunktiven Applikation von niedrig dosiertem Hydrokortison daher nur noch eine Indikation bei Patienten mit volumen- und vasopressorrefraktärem Schock. Mögliche Nebenwirkungen müssen hierbei mit dem potenziellen Nutzen einer Hemmung der Inflammation und Kreislaufstabilisierung abgewogen werden.
