Zusammenfassung

Non-Hodgkin-Lymphome (NHL) und Hodgkin-Lymphome (HL) kommen etwa gleich häufig im Kindes- und Jugendalter vor und machen ca. 14 % der malignen Tumoren dieser Altersgruppe aus. Die Prognose ist mit aktuellen risikoadaptierten Therapieschemata generell als sehr günstig anzusehen. NHL stellen eine heterogene Gruppe maligner Tumoren des lymphatischen Systems dar. Anhand der Einteilung gemäß der WHO-Klassifikation mit klinischen, morphologischen, immunologischen, zytogenetischen und molekulargenetischen Kriterien gelingt die Klassifizierung von >90 % der Fälle. Wie bei den akuten lymphoblastischen Leukämien dominieren die NHL der B-Zell-Reihe gegenüber denen der T-Zell-Reihe. Anders als bei Erwachsenen kommen bei Kindern und Jugendlichen fast ausschließlich hochmaligne, rasch proliferierende Lymphome vor, die generell sehr gut auf eine Chemotherapie ansprechen. Bei den HL liegen deutlich mehr Übereinstimmungen zwischen Kindern und Erwachsenen als bei NHL vor. Diese paucizellulären Tumoren zeigen ein langsameres Wachstum als NHL im Kindesalter. Die Behandlung besteht aus einer kombinierten Chemo- und Radiotherapie in Abhängigkeit vom Therapieansprechen. Bei ausgesprochen guten Heilungsraten rückt eine mögliche Reduktion therapieassoziierter Spätfolgen zunehmend in den Vordergrund. Mit der Einführung von Antikörperkonjugaten und Checkpoint-Inhibitoren ergeben sich neue vielversprechende Behandlungsoptionen.
